My test tag link looked like this when SEF URLs and URL rewriting were enabled: /component/tags/tag/newtag3
After I turned off URL rewriting and turned on the option 'Add Suffix to URL' my tag link looks like yours:
/index/component/tags/tag/newtag3.html
However, the PHP warning your site shows is not displayed in Joomla 5.1.1.
